How to get one or multiple field autofilled by inputting on a field.

Hello,

i'm a newbie, i have almost zero idea about making a form, nevertheless i am trying to make a form. all i need is help from you guys.

i want the Name and Designation get auto filled after i enter the Payroll Id. every time i enter an Id, Name and Designation associated with it get auto filled.

please help.
