How to made PDF interactive

I am attempting to make a PDF interactive. We have a scoring section where the user can press "1, 2, 3, 4 or 5" for multiple questions. How do I make the rate they give us add up to a total at the bottom?

What kind of field are you using for this? A drop-down? Radio-buttons? A text field?
