How to open a pdf form that has to be completed

Download the form to your computer. Open the pdf file in Reader. It is best to use the File -> Open method to open the file, in case you have more than on pdf reading application on your computer. Type in the information requested in the form fields. Save the file, then return it to the people who need the information.
