There are 2 main techniques to send forms by email with Acrobat:

1- use the mailto option or similar scripting techniques but that means the users will need to use their own mail system to process the task

2- use the Distribute function in Acrobat, but it means you know the email address of all your recipients.

You can upload a pdf on your website. Visitor will need to click on a link (text, image, button) to open the pdf and fill it on the screen.

In Acrobat DC, when you edit your form, you can insert a button and set th eproposerties to email you the form (or just the data) back.
