Directly in Adobe Acrobat. In the email is a button which send me directly in Adobe app to the folder,  so I think it’s fine.

The “Approved” status with the name “Anonymous” may depend on how the institution configured its PDF signing or approval workflow. It doesn’t necessarily mean someone accessed your personal information or that the document was approved by an unknown person.

I’d recommend contacting the institution that provided the form and asking them to confirm who approved it and whether the email came from their document-signing system. If the email includes a link, avoid clicking it until you’ve verified that it’s legitimate.

Could you also clarify whether you signed the form directly in Adobe Acrobat or through an e-signature link sent by the institution? That would help narrow down where the status message came from.
