I want to create a form that will allow the user to add an image using an iPad to fill in the form.

I am new to creating fillable forms but have watched the tutorials. I added boxes to the form using the create form wizard where I would like the user to be able to add an image to the form. It works in the preview when using my MacBook pro with Acrobat DC (currently being used as trial), but does not do anything when I click on the field with my iPad.  I saved the form as reader extended pdf enabling more tools. The other fillable fields work, just not the image field.  Suggestions? My goal is to be able to use the form myself to survey the condition of about 100 prints and want to be able to add images to the forms as I go.

The feature doesn't work in the Adobe app for iOS, unfortunately. It does

work in PDF Expert by Readdle, though.

The iOS app - PDF Expert by Readdle works great! It functions perfectly with the form. However, for my purposes - I actually think it will be simpler to not include the image boxes on the form and instead just have designated white space to add an image or images as needed using the app on my iPad. Thank you for your help!
