Images Disappearing on Acrobat Pro After Using ADA Accessibility

I am having an issue with images (usually layered images) disappearing after tagging a document for ADA Accessibility. Here is one example:

When I originally export the PDF, this is what one graphic looks like.

After going through the document and adding tags, the same graphic looks like this:

For other images, they disappear entirely. If I highlight that section, though, it shows the outline of the image, so the data is still there. I tried opening it in a browser to see if it was just an Acrobat problem, but it had the same error in the browser, so the PDF itself has corrupted or isn't reading properly. Please let me know how I can fix that and avoid that issue in the future. Thank you!

@cody_8354 I wonder if it's a layering issue aftr you run the accessibility. After you have accessibility, go to Edit PDF mode and select the artwork and bring to front. Keyboard short-cut, right-click-Arrange-Bring to Front. And also try 'Print to PDF' too

Is the original art an image or vector? How was the original pdf created? I suspect it's an overprinting issue, is the original art an .eps? open it in Illustrator and resave it as a pdf and re-create the pdf (or edit it in Acrobat, use Illustrator to open it and resave it).  Can you upload a sample pdf, before running the accessibility, so we can have a look?
