interactive form output as a PDF.

I want to produce an interactive form output as a PDF.

It has a selection of multiple choice fields that people select and depending on their choice a set of checkboxes automatically fill in. How would I achieve this? What software would I use to originate  it? I guess there may be some background code or links.
