Issues in converting PDF to PDF/X-4

I'm having this issue which is giving me an insane headache. Can any tech wizards here guide me on how to resolve the issue, i have attatched an image below of what results after each attempt.

Help would be HUGELY appreciated, need to convert these pdf's ASAP really

PDF
Print and prepress
PDF forms
Edit and convert PDFs
