Issues with e-signatures

Hello everyone!

I have created a fill in form (for others to fill out) with an e-signature, and this file was saved and is now hosted on a web url.

since october, its been working as intended, users go to the web url, download in some form, e-sign it, then send it back to me.

However, last week, i received 15 submissions where the e-signature shows as it does in the screenshot, with "GA" as the e-signature.

Im not sure if this is a error on the signees end, or what.....Does anyone know what might be causing this?
