Latest Adobe Pro Request for Signature Broken

It seems that the form fields related to checkboxes and dates are no longer working in the latest Adobe Pro request for signatures. Also I have no idea why additional signatures are appended at the end of the documents when I did specify the signatures fields as part of the document.

Can you guys fixed it asap as I have a lot of forms that have been affected?
