Managing created links

Hi, I have a number of links pointing to external files/web pages in a document, and I wanted to ask if there is any way to manage these centrally i.e. if there is any way to see all the links present in a document listed, so that bulk updating them is possible. 

Many of the links I have point to the same documents/URLs, so if there was a way to update them in bulk whenever a target document changes location it would be fantastic.

You can create a plugin for Acrobat.

Not from within Acrobat, no. However, it can be done using a standalone tool, like this one I've developed:

Custom-made Adobe Scripts: Batch Edit URL Links in PDF Files

You can use it to search & replace the URLs of multiple links in multiple files, as well as export a list with all of the current URLs linked to in them.
