Master Collection CS6 Adobe PDF Printer Missing Settings Win 10

Hello,

I have problem with missing setttings in Adobe PDF Printer (CS6, Win 10). I managed to get the first line back, but I can't set the program to ask for the output folder.

 

Any ideas? Repair installation of Acrobat do not solve problem.
