Multiple Yes No Radio Buttons

I have a document that has multiple Yes and No fileds that I need to the Yes or No choice remain on the form until the clients selection is uploaded onto our vendor application. I have Adobe Acrobat Pro version 9. When I created a Radio Button for the Yes or No Field, it doesn't stayy selected when I select another Yes or No Button. My service technitions will have this on their iPads for each customer to fill out and then their selections uploaded into our vendor based application.
