My dynamic stamp is stuck? It doesn't prompt for info any more on new documents.

Hi All,

My collegue has a dynamic stamp. The first time they applied it, it prompted them for info, which they entered. But now, even when they've restarted and are working on a totally new document, it just applies the stamp with that old info, rather than prompting for new info. How do I fix the stamp so that it always prompts for fresh info each time it's used?
