My Fillable forms are auto filling. HELP!

Hi All!

 

I am currently working to put together fillable resume templates. I created the first 2 with no issues -- however, on the new ones - anytime I go to put test information into the fillable fields the program automatically fills in the rest of the fields with the same information. I've attached an example.

 

The virtual assistant on this website has not been helpful. And anything I find in reference to auto-filling is either not related to what I'm doing or it's outdated information...

 

Any help would be appreciated 😀

The bottom three form fields all have the same name. If you change the names of thow bottom two, they will no longer auto populate the data from the first one.
