Named destinations in a Word doc

I have a reference document that was created in Word.  It gets converted to a PDF and posted online.  Hundreds of webpages link to the document, and link directly to the applicable section.  Right now, the links all point to the page numbers.  Because the document is constantly having pages inserted and re-uploaded, this method doesn't work. 

I see that I can used named destinations instead, but it sounds like I have to set those in Acrobat (I'm using XI)?  Since the Word file is the one being updated, I'd have to recreate the named destinations after each revision?  So, is there a way to create named destinations in Word and have them carry over when the document is converted to PDF?
