Need a script for a drop down menu

I'm creating a survey with a drop down menu. What I need is one drop down menu to populate multiple text lines. Knowing I need a script the text lines are just skills1 skills2 skills3 skills4. Preferably would like everytime someone choices and option for it to fill in one of the skills lines.
