Overlapping text in form fields

Hi all,

I am having issues with a PDF form I created. When users are filling out the form fields, several of them are showing duplicate text, overlapping when they save/print. I checked the form for form fields with duplicate names but can't find any. Any idea why the text is duplicating like this and how to fix it?

You created (accidentally, probably) multiple copies of the same field one on top of another. Delete all copies but one.

Here is the file. In form edit view, I'm not seeing duplicate/extra fields, but maybe I am not looking in the right place.

Very strange. Did you open the file in Preview, or some other non-Adobe PDF viewer, by any chance?
