Parental Consent Form - Emails Unknow

Hi,

I want to create a consent form for a team to participation in a competition. As they are youth players they need parental consent. I want to create a form where the player must sign as well as the parent/guardian. I want the form to be accessibly via a link as multiple people will have to complete. For the second signee (parent/guardian) I have to have their email address to put in the form when creating it but I don't want that becasue I want one form for multiple people to fill. Is there a way around the second signatory not requiring an email when creating the web form?
