Yes, this can be done in an accessible PDF.

Requires 3 things:

1. Choose one language and set it as the global (or primary) language of the PDF.  That's set in File / Document Properties / Advanced tab. Toward the bottom, set the main language from the Reading Options section. Example, by setting the global language to Spanish will apply the Spanish language attribute to the entire PDF.
   
   
   
2. Set the secondary language for the text in Urdu. This is done on each tag that is in the secondary language. Select the tag / Right-click / Properties, and choose the language from the drop-down menu. If the language isn't in the menu, type in the language code listed here at ISO 639-2: https://www.loc.gov/standards/iso639-2/php/code_list.php   Urdu = urd, not Urdu.
   
   
   
   If only 1-2 words in a paragraph are in the secondary language, then create a <Span> tag around those words and put the ISO 639 language code on the <Span> tag.
3. Use either a language-specific screen reader, or add a language pack to the screen reader for the secondary language. Check the screen reader manufacturer's website for details.
