PDF Form creation problem

I have created a form and all the form fields are working, until I save it and email it to someone.  Then it is no longer a fillable form.  Same for trying to post the document on a website, the form fields are gone.

Make sure NOT to open the file inside the browser window. Save it locally and open it using Adobe Acrobat or Adobe Reader and it should work fine.
