PDF form gets messed up

Hi,  I made an application form Adobe Acrobat Pro 2020.  It is multiple pages.  They look correct when I send it out to the applicant but when it is returned the format is all messed up.  Example, the graphic and header on page three moves to the bottom of page two.  How to I prevent this and keep the returned document the same as the one I sent out?

Page content shouldn't move around. There are a couple of possibilites.

1.  The viewer used by the recipient is really bad and moving the content. This is highly unlikely and there is nothing you can do about it except tell the recipient to use a real PDF viewer. 

2. These graphic items are annotations and not page content.  In this case the user or the viewer could easily move the items.  The solution is to flatten them into the page content.

I may have figured it out.   I made the form in Adobe but looks like the students are opening it in Google Docs.  I tried it and it distorts it completely.  I don't know if I need to convert the form to Google Docs since 100% of the youth I send it to use it for school.  Disappointed.  Spent a lot of time on this form.

Well that explains it. The PDF is being converted to a different format. Not a good thing. Didn't thing of that one. I guess I'm not young enough 😉
