PDF Module button for exporting result on Server or Sharepoint

Dear Community 

I have some difficulties in find a way to create this workflow :

1. Create a PDF form on Sharepoint/Server  for usage of Employee
2. make it fillable ( done ) 
3. Create a button to upload PDF with a new file name on the server/Sharepoint . 

In the step 3 I stopped because it doesn work the standard option of Acrobat DC and wondering if it's possible with some tips or Javascript specific code.
