Prepare a Form Not Working

I have a PDF form that we use to collect information from people. When I click the prepare a form tool, the tool never enters all the text fields, radio buttons, etc.

 

The form changes every time we use it so we cannot make a template to use over and over. 

 

We used to use a software called Nuance that worked great at this but Adobe Pro does not. Any thoughts.

The PDF form that you are using was created in Acrobat. Please share the PDF form so we can check it on our end. Also, could you share a screenshot of the changes that happen every time?

 

You can try adding all the form fields to the PDF form using Acrobat once and then save it. Then you can use that form as a template.
