Preparing form for multiple pages

I created a form that has multiple pages that needs to have fillable information. The "prepare form" option is only preparing the first page. How do I get it to prepare all pages? There are multiple fields on each page, too many to have to select each item to be fillable.

The Prepare form feature converts all the pages of the PDF to fillable form.

As you have mentioned above, only first page of the PDF is converted. Would you mind sharing the PDF with us so we can check it on our end?  You can share the PDF using the steps provided here https://forums.adobe.com/docs/DOC-7161

It would be helpful if you can share the application and OS version with us. To check the application version, use the steps provided in this link https://helpx.adobe.com/acrobat/kb/identify-product-version.html
