PRINT ICON NOT FUNCTIONING LIKE EXPECTED ON FORM B

I HAVE 2 DIFFERENT CLASS FORMS. THE FORM A WORKS PERFECTLY AND HAS FOR YEARS.

THE FORM B DOESN'T FUNCTION AT ALL PROPERLY. CAN ANYONE ASSIST AND EXPLAIN WHY PLEASE. IT SHOULD WORK UNLESS I AM MISSING SOMETHING NOT SO OBVIOUS.

IF AN ERROR IS FOUND, CAN YOU PLEASE POST A WORKING SCRIPT FOR IT.
