Printing PDF fillable form results in Plugin Error on MAC OSX 10.12

The fillable PDF form in Adobe Reader DC has a Print Button, clicking the print button results in a black screen with the error message  Plugin Error

Opening the form on  Safari using Adobe Reader DC, here is what I believe the partial crash log:
