Based on the error and the workflow, is the file opened on any other application, or is it saved on a cloud drive? 

For files saved on the cloud, there is a high chance that different users will open the file simultaneously, which could lead to an error in saving edits.

If this is not the case, would you mind sharing a few details for further investigation:

1. The app version number you are using (go to Help-> About Acrobat Pro DC);

2. A screen recording video of the entire event to reproduce this in-house.
