Problem with Dynamic Stamp

I have created a dynamic stamp to approve invoices, but it is not working properly.   I have the user input fields created and the check box fields, but when I apply the stamp to a document, the user input fields do not allow for input and the check boxes do not allow checks.

 

When I goto the stamp creation tool, the fields are there and I can add text at that point.  The checkboxes also work there, but when the stamp is saved and applied to a document, the user input fields do not work.  I am sure I am missing something simple, but I cannot figure it out.  PLEASE HELP!!

    The fields must be populated at the moment the stamp is applied (using a script that prompts the user for input).

    Once the stamp has been applied the fields are flattened and the user can't edit them directly any longer.
