Public attendance form

What if i want a sign in sheet laying on a table for a public meeting but its digital instead like on an ipad opened up to a form you just touch sign with your finger and move along for the next person. I haveadded a snip of the form.  very simple i figured if we can do it with a pen and paper why not ipad and finger and digitally save?  is this possible??  i have NO WAY of knowing who is going to show up to these meetings before hand.
