QR code not scanning

Hello, I am trying to generate a form to use as a scannable transfer tag for retail application. I have set up the form and the field to generate the code and it seems to work when i try it with my phone but only up to a certain amount of numbers/letters. Anything more than, say, 10 numbers will not generate a code. Anything less works fine. Help?
