radio button /Checkbox data not populating in PDF response tracker fields

I created a fillable form PDF, which includes a yes/no radio button (alternatively i also created a yes/no checkbox for the issue below) and other text fields. When i distribute and collect the submitted form, all the text fields automatically populate in the PDF response tracker, but the radio button/checkboxes responses are not even showing up in the PDF response tracker fields, despite me select a radio button/checkbox. 

How can i get these radio button fields / text box responses to also populate in the PDF response tracker when i open the submitted PDF file.
