Radio Buttons selections not showing

I am working with Taleo Enterprise. I am trying to create a fillable pdf of the new w4. I have created a radio field on the Taleo Form in the system. I then created a radio field on my pdf using the selections shown. My problem is that the only time i can see a selection is when i select Head of household. If i retest with a different selection, those dont show. I dont know what i am doing wrong and would appreciate some assistance. Thanks! Please see attachment.

Instead of posting a PDF with an image of the file you're working on, post the actual file.

And it seems you're using Acrobat to edit it. If so, please specify which exact version.

Thank you for response. I am using Adobe Acrobat Pro DC 2015 version 2015.006.30508. I dont see a document attachment icon so i cant attach the actual pdf document. what am i missing?

The same way you attached the file to your original post... Or upload it to a file-sharing website (Dropbox, Google Drive, Adobe Cloud), make it public and post the link to it here.

What does that have to do with Adobe?
