Request for Signature coming from "N|pmylett@cpmcomputers.comSignature requested on..."

Question

A client of mine said she received my request for e-signature from a weird address (which I've never seen before) and it sent it immediately to her spam. This is not the first form I've sent to her this way, and we never had a problme. Does this mean something has been hacked?

Here is what it said exactly (minus our document name)

[ ** SPAM ** ]|N|pmylett@cpmcomputers.comSignature requested on "___"

Everything below is looks legit, so it's concerning. I resent the request for signature and it went through correctly. Has anyone else ever seen this happen?
