request image from client via fill and sign document to client

Question to all;  Is it possible to create a form with a image box and have it fill and sign to client, where they would click on image box or action button and allow them to add a photo, then date and sign field to send back?

If you mean using the Adobe "Fill and Sign" service, then the answer is no.  But if you can you sent a regular PDF form with an image field to a client and have them sign it using the "Sign Yourself" tool in Acrobat/Reader, then yes.
