Saving a Fillable Document as a PDF

I am filling in IRS fillable form 1040. When I am done I saved it as 1040-class assignment.

When I go back in to update the new PDF form it is blank with all the fillable areas blank.

How do you save a fillable PDF file so the information you entered stays?
