signature collapsed when use a certificate

Hello experts,

  WHen I tried to "user a certificate" oprton 

my expectaion is put below sign,

but actual sign looks below...

this happened not always, but about 9 times of 10 times happened ( in other words, it success only once a 10 times try)

and I faced this issue over 2 years ago, and still happened in below... 

is there any solid solution ? 
note that, re-stating computer helps to solve sometime, bu only sometims. re-install reader application sometimes solve issue, but only some times...
