Spawn templates from an action button based on conditions

Hello, I am new to Javascript, so I wanted to ask for your wisdom,

I have a 5 page document. First page to enter data and with 4 page templates. The first page asking questions with yes/no radio button. At the end of the questions i want to add an action button named "generate forms" to generate the templates based on the yes/no questions answered. Each YES answer (radio button) needs to be attached to a single template, so if all 4 questions were answered YES, then 4 templates will spawn, and so on.
