Yes, it's called a dynamic stamp, but the user will have to input the data into a dialog window that appears before the stamp is applied.

Once it's applied the form fields in it are flattened and can't be edited directly anymore.

Creating such a stamp is quite a complicated task. There are several tutorials out there, and even a whole book, written about this subject. Also, it requires having Adobe Acrobat. You can't create it using the free Reader.
