Unable to add checkboxes to pdf form

I have enabled the Edit mode for the PDF and have gone to the PDF Form option. The form fields have been generated. I deleted a field and am trying to add a checkbox but am unable to do so. I was able to do this till yesterday evening. Suddenly unable to do it. Can you please provide a solution for this

What do you see when you go to Prepare Form mode (via Tools - Forms)? Edit mode is for editing the static contents of the file, not the form fields within it.
