What would cause the Tab Order to not be respected? (PDF)

I have a PDF form that looks as if the correct Tab Order is set up but when I try to tab throught the form it is seemingly a random order the does not align with the numbering of the Tab Order. What would cause the Tab Order to not be respected?

Any help is appreciated, thank you.
