With fillable PDF, I click "Submit form" and form resets + not filled out when attached to email

I have created a fillable form for one of my clients.  This was their form that I made into a fillable form using my Adobe Acrobat Pro, which they want to email to their partners to fill out and (ideally) email back to them.  I have followed all of the steps regarding creating the "Submit Now" button.  When I fill out the form and click on submit, the entire form resets itself, and the PDF attached to the email is also blank.  Is there a solution for this?    Here is a link to the document, and at the end, there is a submit form button.
