Text to Existing File

My wife has Parkinson's disease, with a very bad memory.  I want to make simple instructions for her, on how to operate the TV, DVD Player & Net Flex, with a Smart TV.

I have a file with the three selectors that do these functions.  I want to add text instructions in this file, with a picture of these three selectors.  How do I do this?  Thanks!
