Can't digitally sign an ADOBE FORM

I HAVE TRIED TO OPEN AND SIGN THIS US ARMY FORM USING BOTH INTERNET EXPLORER AND GOOGLE CHROME BUT NONE SEEM TO WORK!

I CONTINUE TO RECEIVE AN ERROR MESSAGE SAYING "THE SMART CARD CANNOT PERFORM THE REQUESTED OPERATION OR THE OPERATION REQUIRES A DIFFERENT SMART CARD"

ANY HELP WOULD BE GREATLY APPRECIATED!!

As per the description mentioned above, you are getting error message when trying to sign a from, is that correct?

Try filling the form by opening in Acrobat Reader and see if that works.

I downloaded the form from this official site

Opened it in Acrobat and was still unable to sign it ...am I able to send screenshots of the error message in this forum?

You can attach images using the Camera icon.

What happens, exactly, when you attempt to sign the form?

What application, including exact version number, are you using to open it?
