This is a known bug in Acrobat Forms.

And it's sporadic: sometimes a form will print its data correctly, other times not.

2 suggestions:

  • Vote to get this bug fixed at https://acrobat.uservoice.com/forums/590923-acrobat-for-windows-and-mac/suggestions/38809867-form-da...  NOTE: the more votes, the greater the likelihood Adobe will address the problem quickly. Vote! And tell your colleagues to vote!
  • In the meantime, you can save your filled in PDF form as a graphic and it will retain the field data.
    • File  | Save As
    • Choose PNG as the file type. PNG is a graphic file format.
    • Print the PNG on your desktop printer
  • Did I mention to vote to get this fixed? <grin>
