How to do a Course Evaluation Form?

Hi,

We want to know the best way of doing a course evaluation sheet ( happy sheets) which

- don't count towards the users score

- the scores and feedback are recorded

We would prefer also that it can't be done until the course is complete, but this is not crucial becuase we think it's unlikely

Is the best way to use a widget

- to post it as an e-mail or

- access google spreadsheets or

- open a web page where they enter it as form

or

- multi sco

OR

is there a good way within captivate or within an LMS?
