Acrobat Pro Fillable Forms

I created a fillable registration form from a PDF. When I fill it out, save it and email it back to myself to test it is coming back with all the fields blank. Even if I click in the field none of the data shows up. When others test it some forms are coming back blank and others are coming back filled out. What is going on?

    In what app did you fill it? In what app do you view it?

    I created it in Adobe Acrobat Pro DC. That is what I filled it out in and then I had a colleague fill it out and he was using Acrobat Reader DC. Had the problem with both.
