Add a stamp to PDF's to send off for others to complete

We have PDF's that we currently print and stamp to have a different department complete fields on the stamp and return.  How do I make a stamp that would have 5 fields that need data entered in as well as a signature so I can stamp PDF's to email to get the fields complerted and sent back.  Trying to stop the prinitng of paper and just handle these electronically.   I figured out how to make a form but cannot seem to figure out how to make a dynamic stamp that I can pull up and stamp  a PDF then send off to get the data.   Is this possible? 

Our Stamp required the person to give a job #, G/L code, Workorder# as well as  an approval signature/name.

It's not possible to do that with stamps. They are flattened the moment you apply them.

Dynamic stamps contain form fields and a script that prompts the user to fill them in, at the moment they are applied.

Once applied, it's too late. However, if this is something you really want to do, there are ways of adding the fields to the stamp after it was applied, so a different person can fill it in later on, but it would require a more complex script than the ones used for dynamic stamps normally.
