Adobe Acrobat - Answer Selection

I created a document in Illustrator and exported it as a PNG. I then imported it into Acrobat to prepare a form, and Acrobat automatically added fillable fields to my document, which was super helpful. However, I'm running into an issue with the answer selection on a specific set of questions.

The form includes multiple questions about different topics, but they all share the same answer scale: Excellent, Great, Good, Fair, Poor. The problem is, I’m unable to select “Excellent” for more than one question—it only allows me to select “Excellent” on one question. Strangely, this isn’t an issue for other answers like “Great” or “Poor,” which can be selected across all questions.

This limitation is causing issues, as I need to allow respondents to select “Excellent” for multiple answers if applicable. Does anyone know how I can fix this? It’s an important document, and I’d appreciate any help on resolving this issue.

It wont let me deselect because it is a radio box. How do i change it from a radio box to check box

Use squares instead of circles, so Acrobat will add checkboxes instead of radio-buttons.

This is not a good practice, in Illustrator you should : Save as : PDF (uncheck the "Preserve Illustrator Editing Capabilities" option)

I see my new issue. It wont let me deselect because it is a radio box. How do i change it from a radio box to check box


Delete the radio button and create a check box field.
